ODS has several forms which must be completed by students for varying purposes. There are distinctive procedures to follow. Understanding the procedures for the forms is very important. There is a reason for every step in the procedure and every line on the forms.
Assistance Animal Guidelines
University of New Orleans (UNO) is committed to making reasonable modifications to its rules, policies, and practices as required by law to afford people with disabilities an equal opportunity to access its programs, services, and activities.
Emotional Support Animal Guidelines/Documentation Requirements
In accord with the Fair Housing Act, UNO does provide reasonable and appropriate accommodations for a documented student with a disability for an emotional support animal in residence housing. Set forth are specific documentation requirements and guidelines concerning the appropriate use of and protocols associated with emotional support animals. Please contact the office for information on requesting an ESA as an accommodation.
Accommodation Request Procedures for Meal Plan Requirement
In compliance with Section 504 of the Rehabilitation Act and the Americans with Disabilities Act (ADA), University of New Orleans has established procedures to ensure that students with documented disabilities have access to reasonable on-campus meal plan (dining) accommodations. Please contact the office for more information.
On Campus Barriers to Access
UNO is committed to equal access for persons with disabilities. As such, we strive to remove barriers to facilities and programs wherever they exist. You may report the barrier using the form on the link below. Completing the form does not constitute filing a formal complaint or grievance. UNO will follow up in as timely a manner as the information provided permits.
Equipment Loan Form
This form is signed by any student wishing to borrow any piece of equipment (for example digital recorders for use in recording class lectures) belonging to ODS for purpose of accommodation. The form states the individual's responsibility for the care and use of the item, as well as lists the date (and, in some instances, time) by which return of the item is expected. It will be kept on file in the ODS office, and will be presented for the student’s signature by the staff member releasing the item. Student Accommodation Agreement (SAA)
This form is needed each semester for each course for students receiving accommodations. After meeting with each faculty member, the student must submit a signed/completed form for each class in which such accommodations will be needed. This form is also used to schedule tests/quizzes that will be proctored with accommodations in the Accommodative Testing Center.
Students must request their Student Accommodation Agreement Form each semester.
